An Atlas Air B Boeing 747-867F registration N859GT, performing flight GTI95 from Miami International Airport, (KMIA) to San Juan Luis Munoz Marin International Airport (TJSJ) after departure, was on initial climb when flight crew declared MAYDAY, reported engine number fire and requested return back to KMIA.

Date: June 25, 2023.
Location: Palo Alto Airport (KPAO).
A Diamond DA40 Diamond Star registration N660PS, performing flight from Palo Alto Airport (KPAO) to Palo Alto Airport (KPAO) was instructed to hold short of the runway before departure due to approaching traffic but the pilot didn’t stop the aircraft, crossed the hold short line, entered the runway and started takeoff without the clearance.

Due to the inability of the NAV approach instrument, the pilot performed an instrument approach to Runway 8 at Martinsburg Regional Airport (KMRB) using only an iPad, but it died. Therefore, the pilot had difficulty following ATC instructions, resulting in a difficult situation for both parties. The pilot encountered difficulties, but the professionalism of ATC was evident as they effectively managed the situation and gave effective instructions.

A Hawker 900XP, registration N900VA, was destroyed when it was involved in an accident near Westwater, Utah. The captain and co-pilot were fatally injured.
The airplane arrived at West Star Aviation’s facility at Grand Junction Regional Airport (GJT), Grand Junction, Colorado on December 20, 2023 for routine maintenance. According to preliminary information from the maintenance facility, multiple routine inspections had been completed on the airplane including a requirement to remove the wing leading edges and TKS panels to inspect for cracks and signs of corrosion. After the inspections were completed, the airplane was returned to service on February 6, 2024.

Date: January 26, 2024.
Location: Manchester-Boston Regional Airport, NH (MHT/KMHT)

An Ameriflight Beechcraft C99 Commuter (freighter conversion), performing Wiggins Airways flight WIG1046, impacted trees and terrain 70 feet from a single-family home on Colonial Drive in Londonderry, New Hampshire.
The sole pilot suffered serious injuries and the aircraft was destroyed.
The aircraft took off from runway 06 at Manchester-Boston Regional Airport (MHT/KMHT),New Hampshire, at 07:10 LT. ADS-B data show that the aircraft began to follow an erratic course a few minutes after takeoff with varying altitude and speed. The accident occurred 17 minutes after takeoff.

The aircraft, a 1964 Mooney M20E registration N7828V, was at Rocky Mountain Metropolitan Airport at Broomfield (KBJC), it’s still commonly referred to as Jeffco Airport (short for Jefferson County) although it changed its name in 2006
After the incident, The pilot taxied back to a hangar and got out to speak to the airport officers. When the sheriff arrived he immediately noted that the pilot’s eyes were red and watery and he was having difficulty standing, even though he apparently thought he was competent to fly. When he spoke, his speech was slurred and there was no missing the smell of alcohol on his breath.
The sheriff attempted a field sobriety test and then gave up and arrested the man. Back at the station, the pilot completed a breathalyser test which gave a result of a .20 BAC or the equivalent of 200mg per 100ml ethanol in his blood.
